  • Residents with a dementia or mental illness diagnosis (both homes hold DSHS specialty designations)
  • Residents who need Hoyer lift or one-person transfers, gait belt assistance, or a two-hour repositioning schedule
  • Residents with diabetes requiring insulin injections and blood sugar monitoring (via nurse delegation)
  • Residents with incontinence, swallowing precautions, or texture-modified diets
  • Hospice and end-of-life residents, in coordination with the hospice agency
  • Post-hospital recovery and respite stays when a bed is available

Payer sources

Bonney Lake (license 755142): Medicaid accepted, with DSHS contracts for Adult Family Home, Expanded Community Services, and Specialized Behavior Support. Private pay and long-term care insurance accepted.

Buckley (license 759153): private pay and long-term care insurance. No Medicaid contract at this time.

What we are not the right fit for

We do not provide ventilator or tracheostomy care, IV therapy, or complex wound care beyond what home health can deliver in the home. If a referral is outside our scope, we will tell you immediately.

How to refer

  1. Call either home, or send the face sheet, current medication list, and most recent history and physical by fax to (253) 447-6378 (confidential fax).
  2. Our RN owner reviews the referral and calls you back, usually the same day, with a yes or no and any questions.
  3. We complete an in-person assessment at the hospital, facility, or home.
  4. Once the physician assessment, TB screening, and negotiated care plan are in place, we set a move-in date. For pending discharges we can often admit within a few days.
