This website is operated by Silvergate Adult Family Home LLC, 8418 184th Ave E, Bonney Lake, WA 98391. We operate two adult family homes licensed by the Washington State Department of Social and Health Services.

What we collect

When you submit a tour request, question, or professional referral through this site, we collect the name, email address, phone number, preferred home, preferred date, and message you provide. Our web server also records standard technical information such as your IP address and browser type. If analytics are enabled, we use Google Analytics to understand how visitors use the site in aggregate.

How we use it

We use the information you submit only to respond to your inquiry, schedule tours, and follow up about care. We do not sell or rent personal information, and we do not share it with third parties except the service providers that host this website and deliver our email.

Health information

Please do not include detailed medical information in the web form. Clinical records for admissions and referrals should be sent by confidential fax to (253) 447-6378 or discussed by phone. Information about residents and prospective residents is handled according to Washington State law and, where applicable, federal privacy rules.
